Case Management Services

Supportive Housing for Families

This program assists parents in reuniting with children engaged in the foster care system by providing the resources necessary to obtain safe and sustainable housing. The Supportive Housing for Families Program provides the following services:

  • Assists clients in securing housing subsidies and/or affordable housing

  • Transportation assistance

  • Referrals, advocacy, crisis intervention, and counseling services as needed

  • Coordination of substance abuse/mental health treatment

  • Linkage to child-based services, education/vocational support, and parenting resources

Case Management (Region 5)

The Region 5 Case Management Program in partnership with the CT Department of Mental Health and Addiction Services provides case management services to adults with substance use disorders who are prone to chronic relapse and high utilization of community acute care services. In many cases, these individuals experience social, environmental, economic, and psychiatric issues that significantly complicate the recovery process resulting in a need for structured case management interventions. The Region 5 Case Management Team provides the following services:

  • Assistance in accessing local, state, and federal entitlement programs

  • Linkage to community housing, educational and vocational resources

  • Care coordination with community healthcare/treatment providers

  • Advocacy and assistance within the criminal justice system

  • Assistance in completing clinical treatment recommendations and accessing community mental health providers

Our goal is to help our clients identify and secure positive life outcomes by removing barriers through collaboration, advocacy, and guidance.

Transitional Case Management (TCM)

In collaboration with the CT Department of Mental Health and Addiction Services, the Transitional Case Management Program (TCM) seeks to lower recidivism within Connecticut’s correctional system by providing critical support to individuals during their transition back to the community following periods of incarceration. This is accomplished through a combination of comprehensive case management and substance abuse treatment. The Transitional Case Management Program provides support to individuals 3 months prior to and up to 4 months after their release from incarceration. This support includes the following services:

  • Housing assistance

  • Mentoring and guidance

  • Peer Support

  • Assistance in accessing community educational/employment opportunities

  • Advocacy

  • Assistance in accessing local, state, and federal entitlement programs

  • Care coordination with community healthcare/treatment providers

  • Counseling and crisis intervention

  • Linkage to substance abuse/mental health treatment providers
